Background: Acute Respiratory Infection (ARI) remains one of the leading causes of morbidity among children and contributes approximately 3.5% to the global burden of disease. Objective: To analyze the association between mothers’ knowledge regarding exposure to cigarette smoke and the occurrence of ARI among under-five children. Methods: This study employed a cross-sectional design involving 106 mothers with under-five children as respondents. Mothers’ knowledge was assessed using a questionnaire. Data were analyzed using the Chi-square test and logistic regression. Results: Logistic regression analysis showed that the knowledge variable significantly influenced the occurrence of ARI (p = 0.011); [OR = 0.318; 95% CI: 0.131–0.767]. This indicates that mothers with good knowledge had a 68.2% lower likelihood of having under-five children experiencing ARI compared to mothers with poor knowledge. Conclusion: Mothers’ knowledge regarding the dangers of cigarette smoke exposure has a significant effect on the occurrence of ARI among under-five children and serves as an important protective factor. Strengthening health education on the risks of cigarette smoke exposure through community health workers, healthcare professionals, and family-based health promotion programs is necessary. Such efforts are expected to reduce household exposure to cigarette smoke and ultimately decrease the incidence of ARI among under-five children.
